Only a handful of players around the league were extended in 2015. Who signed on and who did not?

Each player drafted in the first round of the 2012 NBA draft was eligible for a contract extension up until Monday, November 2. The Portland Trail Blazers did not come to terms with fourth-year players Meyers Leonard or Moe Harkless by that deadline. Most players did not receive extensions, actually, whether by team decision or their own. Here is the full, 30-player extension breakdown:
